Miracle Prescriptions accepts referrals from hospital discharge coordinators, physicians, nurses, case managers, geriatricians, and community organizations across the North Shore. We provide discharge medication support near Lions Gate Hospital and serve patients in North Vancouver and West Vancouver.

What happens after you refer

  • All referrals are triaged within one business day.
  • Urgent post-discharge referrals are prioritized for same day or next day pharmacist assessment.
  • The pharmacist completes a structured intake with the patient or caregiver, including a full medication reconciliation using PharmaNet and the discharge summary.
  • You receive confirmation when the patient is enrolled, and medication administration records on a weekly or monthly basis as clinically indicated.

  • Medication compliance packaging (pouch or blister) with delivery.
  • Home medication monitoring with nurse visits up to twice daily.
  • At-home medication administration, including insulin support.
  • Post-surgical support and wound care within nursing scope.
  • Opioid agonist treatment dispensing and witnessed dosing.
  • Compounded preparations, including topical pain formulations.
