Medication Administration and Monitoring at Home

Our home program brings the pharmacy to the patient. Nurses visit patients in their own homes in North Vancouver and West Vancouver to administer or witness medications, monitor how the patient is doing, and report back to the pharmacist and care team. Visits are available up to twice a day.

Who the program is for

  • Seniors, particularly those living alone or with limited caregiver support.
  • Patients with memory changes, dementia, or other cognitive impairment that affects medication routines.
  • Patients managing five or more medications at once.
  • Patients with complex chronic conditions such as diabetes, high blood pressure, heart failure, or COPD.
  • Patients recently discharged from hospital who need continued medication supervision.
  • Patients with physical limitations, reduced dexterity, or visual impairment.

What a visit includes

  • Medication administration or witnessed self-administration, based on the care plan and the patient's preference.
  • Help with insulin and other injections, eye drops, inhalers, and patches.
  • Blood glucose and blood pressure checks when indicated.
  • A visit log for every patient, every visit.
  • Concerns observed during the visit are documented and communicated to the pharmacist the same day.

How the program works

  1. Referral or intake. Patients join through a referral from a hospital discharge team, physician, or case manager, or by contacting us directly. Referrals are triaged within one business day.
  2. Pharmacist assessment. The pharmacist completes a medication reconciliation using PharmaNet and the hospital discharge summary where available, and confirms consent and coverage.
  3. Packaging and scheduling. Medications are prepared in date and time labelled pouches, and the patient is scheduled for regular nurse visits.
  4. Ongoing monitoring. Auto-refills keep the medication supply continuous, medication changes from prescribers are applied within 24 hours, and a medication administration record is provided to the care team on a weekly or monthly basis as clinically indicated.

Communication with the care team

We maintain ongoing communication with prescribers, home care nurses, case managers, and family members. Physicians and case managers receive up to date medication records, and symptom or compliance concerns are reported promptly so problems are caught early.
